Facial Plastic Surgery Questions and Answers: Part 6

Question: Will fillers in the cheek and under eye area affect my future rhinoplasty results?
Answer: Fillers placed on the cheeks and under eye area will not have any effect on your nose. Best not to put fillers in the nose itself prior to a rhinoplasty. Choose your rhinoplasty specialist wisely based on extensive experience, since this is the one of the hardest operations to perform correctly in the entire field of cosmetic surgery.

Question: Best procedure for my double chin?
Answer: The Photographs presented demonstrate fat deposits located above and below the platysma muscle in the neck. Liposuction can only accomplish removal of fat deposits above the platysma muscle, while a neck lift surgically removes fat below the muscle in addition to the platysma plasty which significantly improves the jawline. No skin removal is required under age 50. Best to lose the weight first to get optimum results.

Question: Pixie Ear Correction?
Answer: To correct a pixie earlobe requires a minor little procedure at the ear lobe itself performed under local anesthesia. No worries about skin necrosis because the undermining is so minimal.

Question: Is it a brow lift required to achieve symmetry with an upper blepharoplasty?
Answer: While you do have some mild asymmetry of your eyebrows, you are not a candidate for a brow lift procedure due to the normal positioning of your eyebrows currently. A brow lift is only used for patients who have low set eyebrows. With an upper blepharoplasty procedure, you can have a differential amount of skin removed in order to make your eyelids more symmetrical in appearance. In our practice, this can be performed under local anesthesia as an outpatient procedure, which takes about 30 minutes.

Question: Are chin implant or genioplasty the better option? Hoping to improve aesthetics and projection?
Answer: A genioplasty is a major surgery performed by an oral surgeon under general anesthesia in a hospital setting with an overnight stay in the hospital, since it’s more invasive. It’s usually performed when the teeth are significantly out of alignment. Placement of a chin implant is done under local anesthesia, which takes about 30 minutes and it’s much less invasive, and is only done for cosmetic purposes. The size and shape of the implant is usually determined at time of the consultation.

Question: 56, female, how long does a facelift last & how can I improve it’s longevity?
Answer: In our practice, a face and neck lift usually lasts 10 to 15 years. To maintain your facelift results, it’s always best to use sunscreen when you’re outside, and not smoke cigarettes. Mini facelifts to give mini results and don’t last as long. Study your perspective surgeon’s facelift photo gallery to make sure that it is extensive with results that look natural.

Question: Would I benefit most from a neck lift or chin liposuction?
Answer: From the photographs presented, you have significant genetic fat deposits, located both above and below the platysma muscle in your neck. Liposuction alone will only remove the fat deposits above the muscle, and will be only partially effective. You will require a surgical neck lift to remove the fat above the muscle, with liposuction and surgical removal of the fat deposits below the muscle, in addition to a platysma plasty to significantly improve the jawline. It’s also difficult to tell from the photographs if you have a recessive chin, but if that’s the case, then also consider placement of a small chin implant at the same time.

Question: Upper Bleph + Under Eye Filler?

The incision is located directly in your upper eyelid crease.

Question: Is it okay to lose weight after a lower FL/NL slowly? Like 1 pound a week without effecting results?
Answer: It is always best to lose the weight first since you have extensive fat deposits in the neck located both above and below the platysma muscle. The surgical neck lift component can accomplish removal of the fat deposits below the muscle, while Liposuction can only remove the fat deposits above the muscle.

Question: How to diagnose a potential old nasal bone or cartilage fracture?
Answer: Nasal bone fractures heal by adding more bone cells, so an x-ray or CAT scan is not gonna show anything. It’s important to remember that bone is living tissue and repairs itself when there’s an injury. If you have a dorsal hump that bothers you, then you need to have a cosmetic rhinoplasty to remove it. If you have a deviated septum causing nasal obstruction, then you’ll require a septoplasty. Both a septoplasty and a rhinoplasty can be performed simultaneously under one anesthesia with one recovery period.
